Book an appointment with your doctor to discuss your chronic condition. Ask them if you are eligible for a Chronic Disease Management (CDM) plan (this was previously known as an Enhanced Primary Care or EPC plan).

If you are eligible, your GP will create a plan and provide you with a referral form for up to five allied health sessions. Make sure to ask for the referral to be made out to Core Health Group for Exercise Physiology.

Your CDM referral provides a Medicare rebate for up to five sessions with our Accredited Exercise Physiologists per calendar year.

It's important to note that this allocation of five sessions is the total number shared between all allied health services you are referred to (e.g., Exercise Physiology, Podiatry, Dietetics).

During your sessions with us, you will receive:

• A comprehensive initial assessment to understand your condition, capabilities, and goals.

• The development of a safe, effective, and personalised exercise program.

• One-on-one guidance and support from an accredited expert in our state-of-the-art facility

Contact our clinic to schedule your initial consultation. Please bring your referral form and Medicare card to your first appointment so we can begin your treatment.

Please note that Core Health Group is not a bulk-billing clinic. You will be required to pay the full fee for your consultation at the time of your appointment. We can then process your Medicare claim on the spot, and the rebate will be paid directly into your nominated bank account.
